What Happens When You Call Unity?

  • Your call will be answered 24 hours a day, 7 days a week.
  • You will speak with an referral and intake nurse.
  • Details on the reason for your referral including patient dinformation, referral diagnosis and patient health status will be gathered.
  • Unity will offer a free informational visit with the patient and family.
  • Upon determination of patient eligibility, we will call the patient or family to arrange an admission visit.
  • We will inform you by phone within one business day of your patient’s admission status.

Submit an Online Referral: Complete and submit Unity’s Electronic Referral Form and a Unity refferal and intake nurse will contact you within one business day.

Fax Us: Complete Unity’s Hospice and Palliative Fax Referral Form, fax to 920-339-5503 and a Unity referral and intake surse will contact you within one business day.
