Who is eligible?

Treatment Plus is for individuals who:

  • Have a life expectancy of 7 to 24 months
  • May be receiving curative treatment
  • Are not getting enough relief from their medications and other treatment, and
  • Have a minimum of two of the following:

  • Deteriorating health
  • Difficult symptoms
  • Frequent hospitalization
  • Recipient of previous hospice care
  • Declining ability
  • Poor quality of life
  • Treatments may pose burden
  • Recipient of previous hospice care

What is provided?

With Unity, you have a team of experts at your side providing compassion and comfort to help you live each day to the fullest.  Your healthcare wishes and goals will be at the center of your care plan, created in collaboration with your physician and Unity’s care team.

  • Focus of care is optimizing quality of life and reducing hospitalizations through advanced disease management
  • Care team home visits Mon-Fri, 7 a.m. – 5 p.m.
  • Evening and weekend RN phone support
  • Development of personalized healthcare goals based on ever-changing disease progression, needs, wishes and lifestyle
  • Education on disease progression and support for love ones

Where is care provided?

Unity’s care team makes visits anywhere you call home (except a nursing home) throughout our 13 county service area.

At Home

Stay in the familiar comfort of your own home, apartment or a family member’s home.

At Assisted Living Facilities

Receive an extra layer of support at area assisted living facilities.

How is it paid for?

Treatment Plus is covered by private insurance and private pay. Prior to enrolling in Unity’s Treatment Plus program, we will verify your insurance coverage and review the Treatment Plus benefit details with you during an admission visit. As a nonprofit organization, Unity offers CommUnity Care, a financial assistance program ensuring everyone, regardless of financial means, has access to end-of-life care.

If you do not have health insurance, financial assistance may be available. Unity billing coordinators will work with you to determine if you qualify for CommUnity Care.

Unity is the sole provider of inpatient hospice at Bellin Green Bay Hospital, Bellin Oconto Hospital, HSHS St. Clare Hospital, HSHS St. Mary’s Hospital Medical Center, HSHS St. Vincent Hospital, Marinette Aurora Bay Area Medical Center and Door County Medical Center. Unity’s local care teams, coupled with our partnership with area pharmacies and agencies enable us to ensure prompt pain and symptom control as well emotional and spiritual support to patients and families.

Veteran Care

Our end-of-life experts are specially trained in recognizing and supporting the needs of Veterans while promoting dignity, appreciation and respect. Our Veteran-to-Veteran Volunteer Program and other unique Veteran-centered services ensure a meaningful end-of-life journey.

Community Grief Support

Navigating through the grieving process after the loss of a loved one can be difficult. Unity’s grief programs support not only family and friends of Unity patients, but also all members of the community. Unity offers a broad offering of regional grief support groups including Generations, the area’s only support group for children and teens.

Jack and Engrid Meng Hospice Residence

Unity’s Jack and Engrid Meng Residence provides an alternative care option for those seeking either around-the-clock skilled nursing care or temporary caregiver relief. Located on 34 acres in De Pere, Wisconsin, experience compassionate care in a private room.
