Unity Announces Service Territory Expansion Into Entire Counties of Outagamie and Winnebago

De Pere, Wisconsin — Unity, Northeast Wisconsin’s pioneering nonprofit healthcare provider specializing in home care assistance, supportive palliative care, hospice care, and grief support, is excited to announce a territory expansion providing services throughout Outagamie and Winnebago Counties beginning September 1, 2024.

“As the first hospice in the state, it’s been our honor to be part of the fabric of life of Northeast Wisconsin for the past 46 years,” states Unity Executive Director Alisa Gerke. “We’re excited to expand our footprint to enhance the quality of life for more patients and families. Our experienced, local care team knows the community well and is aligned with area pharmacies and agencies to ensure outstanding, prompt, and dignified care.”

Unity matches its full collection of care solutions with the unique needs, circumstances and wishes of each patient and family to create individualized care plans. In doing so, Unity seamlessly transitions those they support to the level of care that maximizes their quality of life, eliminating the anxiety and confusion of moving from provider to provider. The expanded territory enables Unity to support patients and families wherever they call home, including private residences, nursing homes and assisted living facilities, in the following fourteen counties — Brown, Calumet, Door, Florence, Forest, Kewaunee, Manitowoc, Marinette, Menominee, Oconto, Outagamie, Shawano, Waupaca, and Winnebago.
